  • Title of the page 'Services Review'
  • The top functionalities will function similarly to figure 1
  • The content will appear in a style as in figure 2 below
  • The month and year is a drop down selection (Activity review drop-down menu - HF #88 )
  • The back arrow will take you to the page you were on previously
  • Report is generated based on the aggregated values of all CHWs associated with the HF for the first row button, "Review All CHW services", and the individual CHW associated with that row (Service activity counts aggregated for CHW and HF - HF [PLACE] #84)
  • If there are no counts/tallies generated from the app, a default of 0 is assigned
  • All fields are readable
  • There is pagation (Add pagination #81)
  • There is NO Save button.
  • There is NO Confirm button.
  • The look and feel can be similar Stock Usage
