  • Add the Review CHW Services
  • Title is 'Review CHW Services'
  • Back arrow take you back to the last page you were on
  • Below the main title, include the text "Select a CHW to review" as shown in the image below.
  • There will be N+1 rows, where N is the number of CHW associated with this health facility
  • The first row will always be overall aggregations for all CHWs linked to the health facility.
  • The first row text on the far left is, "All CHW services",
  • All other rows will be the CHWs. These will be alphabetical by CHW name or {username} associated with the user. @manutarus @paulinembabu what are the options here and what is the best way to display the user for the combined CHW?
  • All other row text on the far left will be, "CHW {username} services"
  • The arrow on the other side will take you to the individual CHW stock usage information (Create the Services review page - HF #89 )

The wording on the page is different. Should it be "Chwone services" instead of "chwone stock usage"
