You need an API to use Pushbullet. Create account and get your API.

Insert the API key at the Pushbullet page.

You can install the Pushbullet app, the Chrome extension or just use the their website to receive the notifications.

Create multiple templates for different Pushbullet notifications on the Pushbullet page.
