What problem does this feature solve?

目前 create PR 時，缺少固定格式，需自行輸入，導致以下問題

自行輸入特定欄位造成重複繁瑣
PR 可能無法清楚描述問題
PR 格式不固定，造成閱讀不便

What does the proposed feature look like?

新增 PR 模板 : 於 create PR 時，自動套入模板
