Immunization and growth monitoring monthly reports - design

Based on the this workflow in this document, generate a mock-up for the immunization and growth monitoring monthly reports (both draft monthly and sent reports).

@AnnieMungai can you share the original unmarked up screenshots of the above if you still have them? It's probably easier for me to edit on top of those than to find the editable Photoshop files.

@rowo all the screenshots are on these issues. This one for the draft monthly and this one too and this for sent reports. I have also inserted the screen shots
