User story

As a member of the NSF project team, I want to collect feedback from visitors to the beta site so that it can inform subsequent planning and development.

Acceptance criteria

  • Form content and feedback strategy is approved within NSF
  • OMB clearance has been obtained
  • NSF can modify the form as needed
  • [ ]

Definition of done

  • Feedback form is externally available
  • NSF team members can see the feedback as it comes in
  • NSF team members can modify the form
