Insert line doesn't connect all points #16

I am using the reconstruct line in QGIS 3.32.3 and when I select all of my points, click get lines, then insert new lines, not all of the points are joined. Please see attached screenshot. I have several examples of this, is this a bug or a product of the algorithm?

Also it doesn't connect the start or end point.
