This issue pertains to the UI work for the new MyReports page in the PWA and the Physical Activity Report Shell.

MyReports UI

SMDB - Participant Summary Page

• A new row for ROI - physical activity will be added to the SMDB. See requirements doc for full details.
• A link to the physical activity report that the participant receives should be added to the Pt. Summary pg. see requirements doc for full details.
