How do I match clipboard text to fill in a form?

I have a text and I need to remove the unnecessary parts and then fill them into the corresponding positions.
For example:

Name: Anne
Address: No. 116 Old Factory Street
Tel: 87978979

I need to delete Anne, 116 Old Factory Street, 87978979 and fill them in the corresponding form, what should I do?
